Grab a date for your scan by filling out the existing patient form below to reactivate your account! Once you fill this out you will be able to select a date and the location that best fits your needs!
Just like your previous experience with us, your first visit back will be a two day process.
On the first day we will go over any changes that have occurred with your life + health since we saw you last! Then we will have our scan tech update your scans and set you up with a second day appointment.
On that second day we will again review your scans with you and recommend a plan of action to work towards optimal health in regards to your goals! This is when you will have that first adjustment, free of charge!
We know life is busy and that may have been one of the factors for halting care the first time around! We totally get it!
To honor our previous practice members, we have created care options to fit the needs for your health goals combined with your schedule!
These will include 6 and 12 visit options! We will go over these options at your second day visit!
Depending on when we last saw you, we may have new faces on our team as we have expanded in so many ways over the past few years! Including having two locations, we now have FOUR incredible docs on our team who all collaborate and work together to bring you the best care for your family! Every one of our docs brings unique skill sets and personalities to PBD and you will love the collaborative care they provide!
